gsterror.c::_gst_library_errors_init
1) has a duplicate string for
   TABLE (t, LIBRARY, SETTINGS
2) misses docs for (see gsterror.h)
   TABLE (t, LIBRARY, ENCODE
3) should it have DECODE as well?
