RealAudio demuxer, take #2

RealAudio demuxer, works either loop-based with seeking or in streaming mode;
includes documentation; would like to commit directly to -ugly because (a) it
makes sense to have the code close to rmdemux and (b) because rmdemux and
rademux already share some code and should in future be made to share even
more.
