I think this sort of thing happens when the menu is a still frame, which means
there's only one video frame that ought to be displayed, but we get the wrong
one sometimes.

I think it might be caused by QoS messages causing the decoder to sometimes
skip the still frame. Does it happen reliably on that disc, or is sporadic?
That would help confirm it.
