Ok, that means it's not a bug in flacdec then and most likely a duplicate.

Unfortunately I have absolutely no idea where the issue might be. Sounds like a
refcount problem somewhere, but where (and why does it not happen more often)?

It would be interesting to try a pipeline that doen't contain any
basetransform-based elements (audioconvert/audioresample) to narrow it down a
bit. Something like filesrc ! flacdec ! pulsesink or so.
