Finally able to get back to this. I have a set of patches that fix the two
things mentioned in this bug (index contains all tags and index gets written at
the end of file) and a lot of assorted improvements.

Not sure if I should open a separate bug for each one of them... I'll just
attach the whole patchset here and if necessary will open separate bugs.
