I've removed pulseaudio on my ubuntu 12.04 machine (via "sudo apt-get
autoremove pulseaudio") and re-run the test-case. This is the output I got:

Cannot connect to server socket err = No such file or directory
Cannot connect to server socket
jack server is not running or cannot be started
Volume got changed!

So, without pulseaudio and using alsa the callback gets called once (compared
to twice with pulseaudio). But it gets called, into contrast to OS X. So i
think the OS X port needs fixing.
