    mpegtspacketizer: Better detect PCR resets

    Assume that any differences in PCR greater than 15s means there is
    a reset in the stream.

However there's still a problem with timestamp resets in the PES packets, I
have a patch that fixes it almost but has a remaining problem (see the FIXME).
