update segment base time based on stream's segment position

This patch:

- plays correctly the files with multiple segments from bug 345830.
- loops correctly with non-flushing seeks when looping the whole file.

If I loop only until the middle of the video, then it doesn't work.

I am starting to understand a little bit how the segment works, but sincerely,
not fully yet.
