Create pads for text streams

I've been using this patch for some DASH subtitling development work. It works
correctly when playing the test streams here:

http://rdmedia.bbc.co.uk/dash/ondemand/elephants_dream/

(These test DASH streams contain an EBU-TT-D subtitle AdaptationSet.)
