After a long hiatus, here's some code to look at. So far, this provides for
implementing the various frame-packed and stereo modes, and has some
placeholders for doing arbitrary MVC - but I think that'll need some more
exploration as it's actually implemented.

At the moment, I have changes for other modules (libav, ugly, bad) to actually
use the new API - this is just the -base pieces.
