Document Updates for 1.3.0

[Stirling Westrup]

Just wanted to say that I've started rereading the GStreamer documents that
accompany the 1.3.0 git build, and they are HUGELY improved over what came
before.

As someone who actually reads and relies on documentation, I wanted to take
the time to thank those folks who must have worked so hard to add missing
information and to clarify obscure passages.
