Plugins to stream the GstBuffer rwith multi memory blocks

Dear experts,

Is there any plugins which meet the requirements below ?

1. Output h.264 encoded stream
2. Output GstBuffer to the downstream which contains multiple memory blocks

The reason why asking this question is that we would like to check if
our custom h.264 decoder can handle incoming GstBuffer with multi memory blocks.
