ZT-MDP2HD.
Hardware is a Lenovo X1 carbon with:
00:02.0 VGA compatible controller: Intel Corporation 3rd Gen Core processor
Graphics Controller (rev 09)
(via Intel(R) Core(TM) i5-3427U CPU @ 1.80GHz)

Works like a charm and out of the box with current intel hd4000 video drivers
