Note that OpenBox (default config) worked here too if HDMI was configured
right-of VGA, it failed with HDMI left-of VGA.
You need to turn on "Adjust display refresh rate to match video"

Refresh rate will switch if it finds an acceptable match (XBMC needs to be in
