NOT Reproducible with "LibreOffice 3.3.2  – WIN7  Home Premium  (64bit) German
UI [OOO330m19 (Build:202 / tag 3.3.2.2)]" and self created example.doc. Context
menu "Save Picture" works with OOo and OS dialogs.

What does your subject line "No auto refresh" mean?
I do not understand "Button pressed -> nothing changed. But new
folder was created". Creation of new folder seems to be a change?
May I ask you to read  hints on <http://wiki.documentfoundation.org/BugReport>?
Then please decide what additional information, sample document or test kit you
should add to make your problem reproducible.
