Description:
When A TOC is inserted into a two-column page, the Table of Contents, Index or
Bibliography window opens but remains blank and LO hangs up, 'not responding'.
The only way out is to forcibly close it.

Steps to Reproduce:
1. Put the caret at the start of page 2 of the submitted document.
2. Attempt to insert a TOC.
3.

Actual Results:
The Table of Contents, Index or Bibliography window opens but remains blank and
LO hangs up, 'not responding'.

Expected Results:
No lock up!

Locking up like this happens persistently, but I’ve only ever seen it when the
TOC starts on a page with two columns.

In the attached document the default page style is two-column, to demonstrate
the bug. If the default page style is changed to single-column, the TOC can be
inserted.
