Description:
Table of content is broken after copy/paste if it's the first thing on the
first page

Steps to Reproduce:
1. Open the attached file
2. Insert -> Table of content -> Table of content and index
3. Press OK
4. CTRL+A
5. CTRL+C
6. CTRL+V


Actual Results:
The table of content is broken (can't be updated anymore)

Expected Results:
Ideally not


Reproducible: Always
