Okay, it's resolved now. The problem was that I created a new instance of
XRef() for splashOut->startDoc() when I was supposed to use
splashOut->startDoc(doc->getXRef()), instead. Thanks for pointing me to look at
pdftoppm code, that helps!

BTW, I was using the latest stable from the webpage, poppler.0.10.7.tar.gz.
