Test PDF showing the problem.

In the thread [1] opening the resulting PDF created by `texexec --xtx
pdf-fields.tex` (attached) with Evince or Xpdf does not show any input fields
and I get an Error message. It seems to work with the Apple OS and I guess the
poster uses the Adobe Reader or something else.

    $ evince pdf-fields.pdf 
    Error: Annotation rectangle is wrong type
    $ xpdf pdf-fields.pdf 
    Error: Annotation rectangle is wrong type

Is the PDF faulty, i. e. XeTeX or ConTeXt at fault, libpoppler5 (0.12.4-1.1)
too old or something else? I am using Debian Sid/unstable.
