That explains why some of my test files were almost exactly twice as large.

Would it be worth making it a command line option?

I am archiving the generated PS files, so I need them as small as possible.
Someone generating PS files inside a print queue might want them as easy to
interpret as possible even if that made them larger.
It would be similar to size vs. speed optimization options in compilers.

I have some low-end and old printers that often can't print postscript files.
When I have to print a complicated page, I usually have better luck using gs to
send pcl.

Can you attach your pdf file that doesn't print?
I have a way to fix the text positioning in
http://williambader.com/output-pdftopsbug.pdf and I am curious if it works for
your file.
It is based on PSOutputDev::drawString() in xpdf-3.03.
