Uppps: Repair was done with a copy of the XRef (in Catalog constructor!), so
none of the threads got the "repaired" xref. This was a very, very early design
error (when I also wanted to use a copy of the Catalog and not to share the
Catalog).
Quite easy to solve (and works now with the said PDF), hopefully it has no side
effects.</pre>
