In my example the link for A shows at page 2 and top 0, but the pdf viewers
subtract top from page height and they add it to the page offset to find the
target. So, in my example the pdf viewer shows at page 3 instead of page 2.
I think a good fix will be to set changeTop to false if top is 0, so the pdf
viewers will not do this calculation.</pre>
