Use icc profile in OutputIntents

Instead of using static variables I moved now everything to GfxState class
variables in case of icc profile usage from OutputIntents. So different PDFs
with OutputIntents can now be rendered at the same time. I tested it in both
modes, CMYK and RGB, the rendered output is now correct.

Pretty nice side effect: the allocated memory needed for lcms usage is freed at
the appropriate places...</pre>
