On some pdf forms don't work properly with poppler, I have looked everywhere,
but have not been able to find this bug documented, so I am putting it here.
Windows PDF viewers can fill the form just fine, but none of the linux viewers
which use poppler can do it.
I am attaching an example PDF which has this problem. it came from this site:

I am using Linux Mint 17.2 which is based off of Ubuntu Trusty, but I have
friends with Ubuntu 15.10 which also have the same problem. I have not had the
opportunity to test any newer versions of poppler, but being as I couldn't find
any reference to this issue in the bug reports, I am assuming that it hasn't
been fixed yet.
If it is fixed, then please let me know which version of poppler it is working
in and I will try to see what I need to do to get that version running in my
distribution.</pre>
