 I had a case of missing sound device (kernel module was not loaded). I had
 no sound devices configured at all.

 When I tried to play flash video in Firefox, it just quit, with following
 message in logs:

 *** PULSEAUDIO: Unable to create stream. firefox: pcm_pulse.c:115:
 pulse_stop: Assertion `pcm->stream' failed
