#756: PulseAudio should keep the volume settings when disconnecting an USB audio
device

Comment(by coling):

 Note: I use percentages below as displayed in g-v-c - I appreciate %s are
 a PITA regarding actual audio level.

 I cannot reproduce here with pavucontrol.

 gnome-volume-control on the other hand is able to set the volume to > 100%
 (e.g. software amplification) pavucontrol is limited.

 I can reproduce this bug with gnome-volume-control iff the volume I save
 is >100%. e.g. pulse restores the volume but does not honour any software
 amplification.

 Lennart, it's up to the clients to set software amplification AFAIK, is
 there some maximum upper limit in PA? If so, I presume we should limit our
 saved volume to that?

 Also we should standardise g-v-c and pavucontol limits (and my upcoming
 work on kmix) to ensure we have a more or less standard spread of volume
 level upper limits?
