Comment(by coling):

 Some additional notes. It seems that the problem is also related to gnome-
 volume-control-applet!

 If I do the following:
  1. Open pavucontrol and gnome-volume-control
  2. Plug in USB device
  3. Use g-v-c and set device to the USB one.
  4. Use g-v-c and set volume to e.g. 130% (something >100%).
  5. pavucontrol will show volume % but bar will just be at maximum.
  6. Unplug device.
  7. Plug device back in.
  8. Before touching anything, note that pavucontrol has the volume set to
 130% (e.g. it was restored by pulseaudio daemon correctly).
  9. Close g-v-c
  10. Use pavucontrol to set the correct device as default
  11. Note that the volume is reset to 100% Grrr.

 Now if I do the same but:
  9. Close g-v-c
  10. killall gnome-volume-control-applet
  11. Use pavucontrol to set the correct device as default
  12. Note that the volume stays at 130% \o/

 Starting g-v-c-a again and repeating confirms that the volume is limited
 again.

 So as with previous comment, g-v-c and g-v-c-a are almost certainly to
 blame here.
