Comment:

 You are quite right.  Neither of these problems seem to happen with
 Pavucontrol (I assume that Pavucontrol is as reliable as pacmd, when it
 comes to pulse behavior).

 I guess this is strictly a gnome-volume-control problem, working on top of
 Pulse in GNOME 2.28.  I apologize for the noise (I guess I'm pretty rough
 at this bug stuff), and I'll file a gnome-media/gnome-volume-control (I
 think that's the right component) issue with upstream GNOME.

 This wasn't Pulse's fault, sorry for the trouble.
