Comment(by coling):

 Can you attach the output of "pacmd list". There is a bug that sometimes
 prevents the sinks and sources for USB devices from showing up but their
 "card" definitions are properly loaded. I don't think it's related in this
 case, but you never know.

 Incidentally, what distro are you using. I know some Debian variants do
 not deal with ACLs properly on device insertion and thus your user is not
 allowed to access the device and thus PA will not be able to use it (even
 if it does show up in aplay -l). So the output from "getfacl /dev/snd/*"
 would also be useful.
