 Two problems:

 1] The volume change is upwardly un-clamped, i.e. I can use the multimedia
 keys to set the volume to 400% ... 800% .. 1500% ... PA_VOL_MAX
 (probably). I know the default is supposed to be PA_VOL_NORM*3/2 and I
 can't figure out how its getting broken.

 2] Mute doesn't work, i.e. 95/100 times the mute multimedia key quickly
 toggles the volume twice, so as seen in pavucontrol ON->OFF->ON, or
 OFF->ON->OFF.
 I've checked the scancodes via udev and I'm only receiving one input per
 press: /"lib/udev/keymap -i input/event0" -> scan code: 0xA0   key code:
 mute (once).
