The first problem does look like a driver problem. I'm not sure how you could
debug that further.

The second and third problem seem to be setup issues. The third is almost
certainly that Flash isn't going via PulseAudio (via the alsa pulse plugin). It
should "just work" on most distros, though there are manual instructions at
http://www.freedesktop.org/wiki/Software/PulseAudio/Documentation/User/PerfectSetup.
