Jeff, thank you for the bug report. We keep flat-volumes on by default because
we feel that it is the right option for the majority of our users. Of course,
for users such as yourself who prefer to disable this behaviour, the
configuration option is available.

Applications setting their volume at startup need to be dealt with. If you have
a specific list of applications, that would be a good starting point to try to
resolve this.
