I'm afraid I wont be much help as I'm not overly familiar with HDMI
audio, but I would suggest trying to bypass PA and play something via
ALSA directly to test to see if this is a PA problem or something lower
down in the stack.

IIRC it should just be a matter of doing something like:

aplay -D hdmi:0 foo.wav

(although I could be wrong in that - I'm sure a bit of searching can
confirm if I'm talking out of my arse!)
