Here's an updated file.  I've marked all the places out where lists go
as I'm not sure how to do them and most of them seem to be
variable-length and that affects padding as well.

There's also a section on GL non-rendering commands that I have not
added yet.  Should I typedef some new data types as well (like
GLX_GCONTEXTS and GLX_WINDOW, etc.)?  Right now most of that stuff I
left as CARD32.
