[PATCH v2 00/42] Death to extmod

Hi,
A little while later (cough), I've finally finished cleaning up and
rebasing my extmod changes, based on the review comments I got the first
time around.  I'm running this now, and it seems fine -- including having
the Composite visual created in GLX too.

Anything with 'v2' in the subject is new, or has been modified beyond
fixing rebase conflicts.  The rest are (at least as far as I remember)
the same as they were originally.  

The following patches haven't got any review whatsoever: 01, 05, 08, 17,
20, 25, 29, 30, 31, 34, 35, 36, 40, 41, 42 (which is still WIP).
