No way to distinguish user-created modes in RandR 1.2

User created modes are different from server-provided ones; you can
destroy them, and they have to have unique names. As such, it seems like
it might be useful to identify them in the protocol.

I'm thinking we should just add a MODEFLAG that indicated a user defined
mode; that will be entirely compatible with current usage and
implementation.

Any concerns or comments?
