i810/intel drivers with VGA SDVO

Hi

I'm having trouble enabling dual-head (not cloned) video output using
the on-board intel 945G graphics chipset and a plug-in PCIe SDVO card.
Both outputs are analogue VGA.

My target platform is centos 5, I've tried both the i810 and intel
drivers (both are packaged in xorg-x11-i810), with a large number of
permutations of the options for "MonitorLayout".
In all cases the 2nd head fails to come on (no signal).

Attached are my current xorg.conf, and a gzipped Xorg.0.log file from
running it.  I did a quick ubuntu gutsy install and managed to configure
the 2nd head on the fly using xrandr 1.2 so I know that the output does
work.

Can anyone recommend anything other than upgrading X etc. on centos 5?
