In this respin I add some changes per feedback and make the iova entry have proper read/write methods which access pages mapped by amdgpu. So there is no need for /dev/mem or /dev/fmem anymore when reading system memory. Patches 3/4 are unchanged and remove the TTM trace from amdgpu and from TTM itself.
