Sorry for not noticing sooner but in eg cairo-mutex-private.h you have
+#elif defined __OS2__
This should really be
+#elif defined CAIRO_HAS_OS2_SURFACE
as the __OS2__ could break an xlib build on OS/2, especially if using
pthreads.
