[cairo] New Cairo Qpainter surface

Humm, I wonder whether the feature should be called cairo-qt.  Or maybe
cairo-qt-painter or something.  I'm trying to see if the header file should be
cairo-qt.h for everything involved in Qt integration.

Other than that, no objections.  Vlad, can you push it to your tree so I can
fetch post 1.8?
