Okay, I've committed some stuff (hopefully I didn't break anything!). It's not
really much -- I cleaned up the markup a bit, added in labels, changed some of
the copy to make it more friendly, etc.

Still to do:
 * I'd like to try the float left/right thing on "Author" and "Title"
 * The ajaxified keyword inputs

I might get a chance to look at that tomorrow.

Have a few comments:
 * What is the use of "Send File Now"?
 * And what's the use of this: <input name="add_keywords_here" type="hidden" /> ???
