Thanks.  I've rebuilt kernel without IOMMU.

Unfortunately I was not able to reproduce original problem with latest 
kernel - neither with CMA nor without CMA.  System seems stable as a rock.

This is the change on which I've tried to reproduce it with your 
debugging patches:
