Ah, I did just find a several episode DVD in which each episode title belongs
to its own title_set, although from the DVDs I've been testing anyway, this
seems to be somewhat rare.  My patch works fine in this case (and dvdreadsrc
also previously worked in this case too).  Trouble was just the assumption that
a title_set would *always* have a single title.
