add license headers

I copied the licence headers from .h files to their .c file where it was
missing and added LGPL headers to all files that had no license header. I
copied data and names when there was a comment indicating these details.

I also mentioned the files in the REDME which are not used right now (we don't
dist them - should we?).

I also removed filter.c from EXTRA_DIST, as its in SOURCES already.
