The system distribution is Gentoo.

I have absolutely no trace of any pulseaudio in the system.

I was able to fix the issue by lowering the plugins-bad from .16 to .14. So, I
guess the problem is somewhere introduced in between. 

Current versions of the plugins installed on my system is follows:
