This is basically a dump of the changes I've made to osxaudiosrc since my last
major submission in January, but reconciled with the changes made in Git since
then.
The src should try to set the hardware rate if possible.
Multichannel is disabled, because it fails on my system. Maybe there's a way
to fix this, but if it works for nobody then it shouldn't have been enabled in
the first place.
There are also some improvements to the coreaudio buffer creation code for
interleaved vs non-interleaved (in case it ever helps us in future support for
multichannel), as well as some extra debug lines and commented-out code for
future reference.
