I have seen the same problem on Fedora 11. The "flat-volumes = no" does not
really help (well: setting it will totally improve things as the global volume
control logic is totally weird), at least the decrease still happened for me. 

Setting the volume to 100% (as said in the original report) however seems to
work around it.
