New patch to insert the remove silence plugin

The following changes have been made:

- Fixed the headers to be more GStreamer like.
- Added missing change_state function.
- Now reset during state change READY->NULL.
- Fixed the macro name.
- Added the property hysteresis.
- Fixed missing GST_DEBUG_INIT.
- Fixed memory leak because the VAD struct was never being freed (added
finalize method).
