Keep track of segments and synchronize timing to them

With this, seeking inside the video does not cause subtitles to disappear.
Testing with Totem, switching streams (eg, en to fr subtitles) does not work,
and actual languages are not displayed in the selection menu. As far as I can
see, these are the only remaining problems. So far. Which I'll be looking at
next.
