add new REFCOUNT log level and move refcounting there from LOG level

I think in most cases where a *:5 log is required, all the refcounting
information is really not needed at all, and just adds noise. It makes the log
much larger than it already is and harder to read. When playing a 20-minute avi
clip, refcount log messages make up 40% of debug logging lines.

I therefore propose to introduce a new REFCOUNT log category (#7), where we
output recurring refcount log messages into.
